Abstract

The transformation of healthcare services at the primary healthcare level requires the implementation of standardized and well-managed Electronic Medical Records (EMR). One of the major challenges in EMR implementation at community health centers is the absence of written guidelines to support registration and EMR management processes. This community service program aimed to implement an Electronic Medical Record Guideline Book as a strategy to improve service quality and operational effectiveness at Puskesmas Pandanwangi. The methods included problem identification through observations and interviews, priority setting using the Urgency, Seriousness, and Growth (USG) method, root cause analysis using a fishbone diagram, development of the guideline book, and dissemination activities for medical record staff. The USG analysis identified the absence of EMR registration and management guidelines as the highest-priority problem. The guideline book was developed according to the conditions and needs of Puskesmas Pandanwangi and covered registration workflows, EMR management procedures, staffing standards, occupational safety, and quality control. Implementation was carried out through training and dissemination sessions, followed by monitoring and evaluation activities. The results showed improved staff understanding of EMR registration and management procedures, as well as the availability of standardized work guidelines. The implementation of the Electronic Medical Record Guideline Book is expected to support administrative compliance, reduce documentation errors, and improve the quality of healthcare services at Puskesmas Pandanwangi.
