ANALISIS DETERMINAN KEJADIAN MISFILE REKAM MEDIS DI RS BHAYANGKARA BONDOWOSO

Misfile incidents represent a significant issue that directly contributes to service delays and reduced quality in medical record management within hospitals. This study aims to analyze the determinants of misfile occurrences in medical records at Bhayangkara Bondowoso Hospital using the 5M approach (Man, Method, Machine, Material, and Money) and to identify priority problems through the USG method (Urgency, Seriousness, Growth). This qualitative research employed observations, in-depth interviews, and document analysis. The results showed that during the period of 8–13 August 2025, there were 40 misfile cases out of 1,611 retrieved files, with an average incident rate of 2.48%. The contributing factors included inadequate competencies among filing staff, limited budget and storage space, the absence of color-coding on medical record numbers, non-compliance with standard operating procedures (SOPs), and inconsistent data entry of medical record numbers in the hospital information system (SIMRS). The USG scoring identified three main priorities: the absence of color-coding, inadequate shelving capacity, and inconsistent SIMRS data entry. The study concludes that implementing color-coding, optimizing storage facilities, enhancing staff competencies, and strengthening SOP compliance and SIMRS documentation are essential to reducing misfile incidents and improving the quality of medical record management.
